We provide individual health services for people with any type of behavioral health concerns such as substance abuse, emotional issues, and intellectual and developmental disabilities.
Our services are open to anyone in Monroe County who does not have insurance and cannot pay, or are insured by Medicaid, MIChild, or Health Plan of Michigan (HPM).
Only those with Medicaid are guaranteed services.

DESCRIPTION Under the general supervision of the Programs Supervisor, the Client Services Manager (CSM) assesses the needs of the consumer and the consumers family, when appropriate, and arranges, coordinates, monitors, evaluates, and advocates for services to meet the specific consumers needs through person center planning (PCP) and the development of an individual plan of service (IPOS).
The Client Services Manager is responsible for addressing both the individual consumers biopsychosocial status as well as the state of the social system in which case management operates, assuring the implementation and delivery of services to individuals and their families in keeping with the principles of recovery, self-determination and resilience, as well as developing and maintaining a therapeutic relationship with the consumer.
CSMs are also responsible for linking consumers with external systems to provide additional services, resources, and opportunities consistent with qualitative and quantitative performance goals and standards established for the Client Services Management Program.
In addition, Master level case managers may provide therapeutic services to consumers in addition to case management services.
This position is responsible for providing Case Management services to adults determined to have long term mental illness and a developmental disability which impedes adequate functioning in one or more of the following areas: school, work, community, family, and interpersonal relationships.
POSITION REQUIREMENTS Education, Training and Licensing Credentials: Bachelors Level (Pay Grade 7) Bachelor's Degree in Social Work with State of Michigan Licensure.
Minimum of a Bachelors degree in social work is required.
Appropriate Michigan licensure/certification is required.
Masters Level (Pay Grade 9) Master's Degree in Social Work, Psychology, or Counseling with State of Michigan Licensure.
Masters level such as Licensed Masters Social Work Limited License Psychology Licensed Professional Counselor Experience: B.

CSM One (1+) or more years experience within the specific consumer population in which the position holds responsibility (adults with mental illness (QMHP*), adults with developmental disability (QMRP**), integrated care, assertive community treatment) preferred.
Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.
Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individuals status; interpret the appropriate information needed to identify each individuals requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group (adult and geriatric).
One year experience with the chronic mentally ill and vocational education preferred.
Willingness to receive SUD training and obtain certification (CAADC, CADC, CCJP, CCDP, or CCDP-D).

*QMHP - Individual with specialized training or one year experience in treating or working with a person who has mental illness as defined under the Michigan PIPH/CMSP Provider Qualifications per Medicaid Services.
** QIDP - Qualified Intellectual Disability Professional (QIDP) - Individual with specialized training or one year experience in treating or working with a person who has intellectual disability; and is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, registered dietician, therapeutic recreation specialist, or a licensed or limited-licensed professional counselor.
One year experience with the chronic mentally ill and vocational education preferred.
Willingness to receive SUD training and obtain certification (CAADC, CADC, CCJP, CCDP, or CCDP-D).

A related degree and /or a limited license may be considered in lieu of the above where provider qualifications as defined under the Michigan Mental Health Code and/or Medicaid Provider Manual and appropriate supervision exist.
Other Competencies and Abilities: Ability to work in a complex, detail-oriented, deadline structured busy organization.
The following physical and mental abilities are required: frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking going up and down stairs due to community outreach basis, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write reports, correspondence and procedures, ability to maintain consumer charts/records ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others, and the ability to conduct patient assessments and treatment.
Demonstrated proficiency in MS Outlook, MS Office, electronic medical records (CRCT) and such other applications if/when required.
Ability to be trained and successfully complete other computer training as required.
Valid drivers license, proof of current automobile insurance and driving record determined by the Authoritys insurance carrier to be eligible for coverage if required as part of positions duties.
